Pre-Registration Requirements

These are the forms you will need to complete prior to bringing your child to the Under the Weather Childcare Center. These forms will only need to be completed once and you may either bring them in the first time you visit the center, or fax them to our office before your visit. The Child Immunization and Healthcare Summary forms may be requested from your healthcare provider or your current daycare provider. They may fax the forms directly to Under the Weather at 763-425-5929, or you may pick them up and bring them with you. The samples below, numbers 3 and 4, show what these forms look like.

Most of these forms are available both as Microsoft Word documents and in Adobe Acrobat format.

1. Under the Weather Child Admission Form
       Enrollment form and Emergency Contact Information.

3. Child Immunization Form
        Required by the state for all children attending daycare facilities. This form is only available in PDF format.

         This form is to be filled out by your Healthcare provider.
